Alcohol Approval Request

IMPORTANT BEFORE YOU PROCEED:
  • If your event is in Smith, Viking Pavilion, or Lincoln Hall, you must use PSUEats to serve alcohol. Before you fill out this form, please contact them to talk through your event and catering needs.
  • If your event is outside those buildings, you are encouraged to use PSUEats. If you would like to use an outside caterer, you will need to have all the business information available to complete this form including catering menu and the Health License, Liquor License, Certificate of Insurance, and a Food Handlers Card from the catering company. Again, using PSUEats is heavily encouraged to avoid these extra steps.
  • Due to liability concerns, PSUEats will not serve alcohol or food for an event that also has an outside caterer.

All alcohol service must be approved by the University. If you wish to serve alcohol at your event, organizers must submit an Alcohol Request Form at least 15 business days prior to the event. When approving your event to serve alcohol, the University is working to make sure that you are meeting university policy, Oregon Liquor Licensing policy, and our catering contract obligations.
